- [Under The Muscle Breast Implant](https://www.drweinrach.com/under-the-muscle-breast-implant/): Placement of a breast implant under the muscle or on top of the muscle is a topic frequently asked about and always discussed during a breast augmentation consultation.  The muscle that is being referred to is the pectoralis muscle.  This is a muscle on the chest wall that is on top of the ribs.   A breast implant can be placed either on top of this muscle or underneath this muscle.  Placing the breast implant underneath this muscle has several advantages.  When the implant is under the muscle, there is more soft tissue (the pectoralis muscle) over the implant.  This gives the breast a more natural look and feel.   The second and very important advantage is that a breast implant under the muscle has a much lower risk of developing a capsular contracture when compared with a breast implant placed on top of the muscle.  Finally, when a breast implant is under the muscle, it is easier to perform and read mammograms then if the implant is on top of the muscle.  While placing a breast implant on top of the muscle is probably easier to perform, it has the disadvantages of a less natural look and feel, much higher capsular contracture risk, and causes more difficulty with mammograms.   For these reasons  it is preferred to place the breast implant under the muscle in most cases.   Situations where placement of the breast implant above the muscle is considered include cases when a woman already has a breast implant above the muscle, and desires exchange for change of size or change of filler (saline to silicone gel).   Even in these cases the implant location can be changed from on top of the muscle to underneath the muscle if desired.   These issues are best sorted out with a thorough in person consultation with a board certified plastic surgeon.

Shaped, anatomic, or tear-drop implants may be better for women who desire a breast augmentation with less upper pole fullness than a round implant might provide. Texturing was developed to try and lower the chance of developing a capsular contracture after a breast augmentation.  A capsular contracture is basically a firm scar that can develop around an implant. While all breast implants will develop a capsule, normally the capsule is thin and soft. A capsular contracture is what everyone would like to avoid. One very good way to lower the chance of getting a capsular contracture is to place the breast implant under the pectoralis muscle. Why this placement lowers the chance of developing a capsular contracture is not clear. Textured implants have definitely been shown to lower the chance of developing a capsular contracture when placing a breast implant above the muscle. The question is does placing a textured implant under the muscle lower the chance of developing a capsular contracture when compared with a smooth implant under the muscle. This is not very clear yet, however there is a small amount of preliminary data that possibly demonstrates a small benefit. The breast implant company Sientra warrants their implants against capsular contracture - should a primary breast augmentation patient develop a significant capsular contracture with a textured Sientra breast implant, they will issue a new implant at no charge.

This is a very difficult question to answer over the phone for several reasons. The main determination of how much a procedure will cost is how long the procedure will take to accomplish. In a nutshell, the longer the procedure, the more it will cost, as anesthesiologists and operating room personnel all are paid by the hour. Also, longer cases use more anesthetic medications compared to shorter ones, which increases the cost of the case. More materials are needed for bilateral (both sides) cases, compared to unilateral (one side) cases. A "tummy-tuck", also known as an abdominoplasty, may be a relatively short procedure in one person with very little excess skin on the abdomen, or quite a long procedure in a person with a very large amount of excess skin on the abdomen and flanks. The best way to determine how much a procedure will cost is with an in person consultation. This will allow Dr. Weinrach to examine you and accurately determine which procedure or procedures would be most appropriate; the length and complexity of the procedure will determine the cost.

The other great benefit is that the funnel protects the implant from ever contacting the skin as it travels into the pocket; no skin bacteria can come into contact with the implant. Women with very significant amounts of excess skin and sagging usually require a formal mastopexy, or breast lift, in order to improve breast contour. The tradeoff for this improvement in contour is scars on the breast. Some women do not mind having scars on their breasts in order to have improved contour, but other women can not tolerate significant scarring on their breasts. In women with small degrees of sagging, or ptosis, a significant improvement in breast contour can often be achieved with a breast augmentation using an implant, without the additional scarring associated with a formal mastopexy.  A modification in technique of traditional under the muscle breast augmentation is required in order to achieve this, but in the right patient a nice result can be accomplished:

There are twenty four medical specialty member boards (such as cosmetic surgery boards and American plastic surgery boards) recognized by the American Board of Medical Specialties (ABMS), the preeminent entity overseeing the certification of physician specialists in the United States. The American Board of Plastic Surgery is one of the twenty four recognized members of the American Board of Medical Specialties. The American Board of Cosmetic Surgery, which came into existence in 1990, is NOT recognized by and is NOT a member of the ABMS. The American Board of Plastic surgery was organized in 1937 and became recognized as a member board by the American Board of Medical Specialties in 1941. Becoming a surgeon certified by the American Board of Plastic Surgery is a lengthy and rigorous process: After graduating from an accredited medical school, one must complete a minimum of 5 years of surgical residency training, with at least 2 years devoted completely to Plastic Surgery. These residency training programs must be recognized by the American Board of Plastic Surgery. Many Diplomats have undergone extensive training well beyond the minimum. In addition, rigorous and comprehensive written and oral examinations must be passed. This information can be verified and more elaborated on at the American Board of Medical Specialties and the American Board of Plastic Surgeons.
- [Shorter breast augmentation incisions with the Keller Funnel](https://www.drweinrach.com/shorter-breast-augmentation-incisions-with-the-keller-funnel/): In the past, one of the disadvantages of a silicone gel breast implant for a breast augmentation was the need to make a longer incision to place it compared with a saline implant. This is because a gel implant is placed completely filled, as opposed to a saline implant which is placed empty, and then filled after placement. By using a Keller Funnel, most silicone gel implants can be placed through an incision that is the same length as the one used for a saline implant. This results in a shorter, less visible scar.  Because the implant never touches the skin when the Keller Funnel is used, there is also less chance of bacterial contamination of the implant, which likely equals a reduced chance of breast implant infection. Although the cause of capsular contracture is not known, a widely accepted theory is bacteria; the use of a Keller funnel may greatly lower the possibility of a capsular contracture as well.

- [Choosing Breast Implant Size](https://www.drweinrach.com/choosing-breast-implant-size/): An important issue when deciding to have a breast augmentation is choosing a breast implant size. There are many factors that influence the choice of the appropriate size breast implant for a particular woman. Many women have a friend who has already had a breast augmentation, and think that they want the same implant size that their friend has, based on what their friend’s breasts look like. The problem with this approach is that the final size of the breast is a combination of the implant PLUS the amount of tissue that one is starting with. If two different women are starting with two different sized breasts, than placing the same size implant in both of them clearly will result in different sized breasts postoperatively. There are other important anatomical issues to consider which vary considerably from one woman to the next. One is the starting volume of skin. A woman with more breast skin can accommodate a larger implant than a woman with less breast skin. Another big issue is tissue quality. A woman with thinner tissue will tolerate much less expansion in size than a woman with thicker tissue. The larger the implant, the more the tissue is stretched and thinned, resulting in less coverage of the implant and a less natural result. Choosing breast implant size also involves various measurements that are taken of the breast during a breast augmentation consultation. These measurements vary greatly from woman to woman, and are important in selecting an appropriate size and profile of breast implant. At times, some women prefer to have a larger implant placed than what may be ideal for her anatomy, accepting the trade-off of a larger size for a less natural feel. A consultation with an experienced board certified plastic surgeon, certified by the American Board of Plastic Surgery, can analyze all of these issues and produce the best possible outcome after a breast augmentation.

This was corrected with otoplasty surgery. A better upper ear fold was created for him using sutures hidden through an incision at the back of the ear. The ear was also set back with a combination of removing a small amount of his extra ear cartilage, as well a suture to move the ear back into a more normal position. His earlobes were also moved back slightly with carefully placed sutures. There are no incisions or scars visible - all work is done at the back of the ear, hidden in the natural crease. As you can see, he is very pleased with the result.
