What is a Breast Augmentation?

Breast augmentation, also called augmentation mammaplasty, is the most common cosmetic surgical procedure performed. There were 290,000 breast augmentations performed in the United States in 2016. Usually breast augmentation is desired simply because a woman feels that her breasts are and have always been too small. Another common situation is loss of breast volume after pregnancy, and breast augmentation is done to restore the breasts to their pre-pregnancy volume. Some women have significant asymmetry – one breast is much smaller than the other. Placement of a breast implant in the smaller breast or sometimes in both breasts can make the breasts more equal.

Using either saline or silicone gel breast implants, Dr. Weinrach is able to increase the size of the breasts by one or more bra cup sizes. Your individual anatomy, skin quality, and desires will all influence the decision regarding the size of the implant that will be placed.

Are you a good candidate for Breast Augmentation?

Good candidates for breast augmentation are healthy and have realistic expectations about what breast augmentation can accomplish. Some very positive effects of breast augmentation, in addition to simply increasing the size of the breasts, include improvements in self confidence, and giving the appearance of a smaller waist. Many women also report they have a much easier time shopping for clothing that fits well when their breasts are more in proportion to their bodies.

Breast Implants

There are many different styles and shapes of breast implants available. Breast implants are filled with either silicone gel or saline (salt water). Both have a silicone outer shell. The main advantage of the silicone gel filled breast implants are they feel much more like natural breast tissue than saline implants. The main disadvantage of silicone gel breast implants when compared to saline breast implants is that they are more expensive. Saline filled breast implants on the other hand are much less expensive, but they do not feel as natural as the silicone gel filled breast implants. In the past, a longer incision was required in order to place a gel breast implant, however now a silicone gel breast implant can be placed with a much shorter incision by using a Keller Funnel. In some patients with very good quality breast tissue with adequate volume to begin with, a breast augmentation with a saline breast implant can feel quite acceptable. There are a variety of of available breast implant styles and shapes, and three different breast implant companies that manufacture them. Currently, the three breast implant companies are Sientra, Mentor, and Allergan. Each breast implant manufacturer has its own proprietary silicone gel filler. Modern silicone gel filler is usually described by “cohesiveness”, which really refers to the ability of the silicone gel filler to resist changes in shape. Modern cohesive gel breast implants are sometimes referred to as gummy bear implants. Breast implant shells can be either smooth or textured, and implants can be round or shaped. Breast Augmentation Evaluation and Consultation

Dr. Weinrach will review your medical history and examine you. In order to select the most appropriate breast implant, several measurements will be taken of your breasts. Dr. Weinrach will also have a thorough discussion with you to determine your particular concerns and expectations. Then, based on your concerns as well as the measurements taken, implant sizers will be selected for you to try on in the office. The quality and tone of your breast tissue, which is different from patient to patient, is also an important contributor to the final outcome. The combination of tissue quality, measurements, and personal desires makes each case unique; each of these issues will be addressed when selecting the proper implant for you.

Where Your Breast Augmentation Will Be Performed

Dr. Weinrach performs breast augmentation only in fully accredited surgery centers in the Scottsdale, Arizona area. Breast augmentation is nearly always done as an outpatient procedure – no overnight stay is usually required. You will be able to go home the same day.

Anesthesia for Breast Augmentation

A board certified anesthesiologist will administer anesthesia for your breast augmentation. You will be completely asleep, and feel nothing during the breast augmentation procedure. The anesthesiologist will monitor you very closely during the entire procedure. Dr. Weinrach also places local anesthesia during the procedure to help minimize discomfort after surgery.

How a Breast Augmentation is actually done

An incision in the skin is made to create a space under the breast for placement of a breast implant. There are several locations that can be used to place this incision, including underneath the breast where the breast meets the chest, the lower border of the areola and skin, or the armpit. There are advantages and disadvantages of each of these locations which will be discussed during your consultation. A pocket is then created either underneath or on top of the pectoralis muscle for placement of the breast implant.

Placing the breast implants behind your chest muscle has several advantages. This location gives more soft tissue coverage over the implant, resulting in a more natural feeling breast. This location also seems to greatly lower the possibility of developing a capsular contracture. Mammograms are easier to perform and read with implants placed underneath the muscle.

Breast augmentation surgery usually takes about one hour to complete. Absorbable stitches are used to close the incisions. There are no stitches to take out.

Recovery after Breast Augmentation

Discomfort from breast augmentation surgery is usually very easily controlled with over the counter anti-inflammatory medicine and sometimes a small amount of prescription pain medicine. Most patients describe only either mild “soreness” or very little to no discomfort at all. You will be up and walking the day of your surgery. Some patients even go out to a restaurant for dinner that evening.

A sports bra is usually worn for about 6 weeks after breast augmentation surgery. There will be mild swelling in your breasts will take several weeks to resolve. The majority of patients return to work with a few days. Occasionally the time to return to work could be longer if a particular job requires very strenuous physical activity or heavy lifting.

Dr. Weinrach will give you instructions regarding exercising and resuming normal activity. In general light activity is advised for several weeks.

With time, your scars from the incisions will begin to fade. Most women are amazed at how hardly noticeable their scars eventually become.

Results after Breast Augmentation

After surgery, most women are very thrilled about their new, fuller body and appearance as well as their ability to fill out new clothes in a way they could not before. Regular examination by Dr. Weinrach and routine mammograms for those in the appropriate age groups are important and recommended.
