Appointments

Please call us at 480-634-6014 to schedule your appointment. If you prefer you may contact us here to request a call back. Please include your phone number in your contact form.

A drivers license or other valid government issued picture identification will be required at check-in in order to become a patient of Weinrach Plastic Surgery.  A picture of your face will also be taken to become part of your private electronic medical record.

Cancellations

If you are unable to keep your appointment, we ask that you kindly provide us with at least 24 hours advance notice. This courtesy on your part will make it possible to give your appointment to another patient.

Children

Although we adore children, unless a child is the patient, our office is not the best place for them to be. Medical equipment, sharp instruments, and high voltage can present dangers to children. Children can also be a distraction during the consultation process, making it very difficult to process a lot of new and important information. Children under the age of 10 are strongly discouraged from being in the office. We can make accommodations for a young child if the child is the patient. We apologize for any inconvenience that this causes, but we feel it is in the best interest of everyone (especially the children).

We are open Monday – Thursday 9:00am – 5:00pm

Friday from 9:00am – 4:00pm

Billing and Payment

The consultation fee for cosmetic surgery and procedures for new patients who have not had surgery or a procedure on the area of interest within one year is $75.

The $75 consultation fee will be applied toward the surgery.

The consultation fee for cosmetic surgery and procedures on areas that have been treated within the past year by another practitioner is $150. 

The consultation fee for breast revision and breast implant removal consultations is $150.

The $150 consultation fee will be applied toward the surgery.

Payment in full or financing arrangments with Carecredit or PatientFi is due two weeks prior to any surgical procedure.

For credit card payments greater than $500, the person’s whose name is on the credit card must be physically present along with his or her driver’s license or other valid government issued photo ID.

We can not accept checks.

Recovery from surgery and FMLA

Cosmetic plastic surgery does not qualify as a serious health condition under the Family and Medical Leave Act (FMLA), part (d): https://www.govinfo.gov/content/pkg/CFR-2016-title29-vol3/xml/CFR-2016-title29-vol3-sec825-113.xml.  Due to this federal law, we are not able to complete FMLA forms for time off work.  Arrangments for time off work must be made between you and your employer.  Dr. Weinrach will discuss activity restrictions with you which depend on your procedure.
